Position Summary

In accordance with the established mission, vision, values, and strategic priorities of Northern Health and under the general supervision of the Manager or designate, the Social Worker is a member of a multidisciplinary team that provides youth and their families with mental health and substance use treatment and supports. The primary responsibilities include community engagement and outreach services to at-risk youth or identified as at-risk, by providing assessments, early interventions and referrals for substance use and/or mental health problems. This position will work in settings where contact with youth at risk can best be established, e.g., street-based, school, and community settings. Providing mental health and substance use services where the focus is maintaining clients in the community as well as enhancing youth treatment and rehabilitation plans through regular and supportive contact. As a member of a multidisciplinary health care team, providing direct community based early intervention, crisis response, assessment and treatment, planning and implementation of the acute discharge plan, and ensuring youth have a seamless transition between services. The Social Worker, Y-Star, works within the framework of Harm Reduction in substance use, promoting the benefits of this approach in collaborative work with collateral service providers.

Shift Rotation/Hours of work: Days - 10:00 to 18:30, Rotating

Qualifications

• Bachelors Degree in Social Work from an accredited university.
• Two (2) years' recent, related experience working with youth with mental health and substance use challenges or an equivalent combination of recent, related education, training, and experience.
• Current full registration with the BC College of Social Workers.
• Valid BC Drivers’ license – use of personal vehicle may be required.

• Demonstrated ability to communicate effectively with people and their families, the public, medical staff, and members of the interdisciplinary team. Ability to effectively resolve interpersonal differences and resolve conflict.
• Problem solves demonstrating critical thinking and decision-making skills.
• Ability to promote person centered care which includes maintaining sensitivity to diverse cultures and preferences, social justice concerns and the need for advocacy.
• Demonstrated ability to organize work, set objectives and establish priorities. Manages time and resources, implements activities to promote cooperation among the interdisciplinary team and collaborates across disciplines.
• Demonstrated ability to foster teamwork and a commitment to excellence in the provision of person-centered care.
• Demonstrated computer skills including the use of Windows based programs. Ability to operate other related equipment as required in the specific practice area.
• Physical ability to perform the duties of the position.
