Do you have a passion for nursing? We have the position for you! We are seeking a motivated Licensed Practical Nurse with a dedication for delivering compassionate patient care to join our team in Primary Care.
In accordance with Northern Health’s vision of an idealized system of services built on a foundation of Primary Care where people and their families receive primary care services in Primary Care Homes supported by interprofessional teams.
The Primary Care LPN functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities. The Primary Care LPN provides care that includes clinical follow up according to the person’s primary Care Plan and the person’s personal goals of care to inform plans of care developed collaboratively with the interprofessional team. The Primary Care LPN will support people and their families to achieve and maintain optimal health.
The Primary Care Licensed Practical Nurse performs assessments, plans, and implements and provides personal and nursing care to people. The LPN administers oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures. The scope or extent of the skills that the LPN performs varies according to the predictability of the outcome for a given population. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the BC College of Nurses and Midwives (BCCNM) and according to organizational policies, procedures, standards and unit specific protocols.
Primary Care Networks are partnerships between the Divisions of Family Practice representing BC’s family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign B.C.’s primary and community care system so that it works better for patients and health-care providers.

Qualifications
• Graduation from a recognized program for Practical Nurses with one (1) year of recent, related experience; or an equivalent combination of education, training and experience.
• Registration with BC College of Nurses and Midwives as a practicing LPN registrant (BCCNM).
• Current, valid BC Driver’s License.
Skills and Abilities:
• Demonstrated knowledge and skills within the LPN defined scope of practice or patient population dependent on the nature of the caseload (e.g. acute care, gerontology, palliative, etc.).
• Ability to communicate effectively, both verbally and in writing.
• Ability to establish and maintain rapport with people and their families.
• Ability to work independently and collaboratively with others within the context of an interprofessional team.
• Ability to observe and recognize changes in a person’s condition or health status.
• Ability to plan, organize and prioritize; and ability to implement a plan of care.
• Ability to operate related equipment including proficiency in relevant computer applications.
• Physical ability to perform the duties of the position.
